The easiest way to become a member is to go to www.mendedlittlehearts.org.

Click on How to Become A Member. There is a form that you will need to fill out. Make sure that you select "Mended Little Hearts of Dayton (Dayton, Ohio)" from the Group drop-down list. Make sure to click on the Submit button located at the bottom of the page, after you have completed the form.
